Registration Checklist for the Medical Interpreter Program
Thank you for signing up. Please complete the checklist before you start the course. Current or returning students are required to complete the checklist every three months.

I certify that I can speak English and my non-English language fluently, and able to read and write in both languages beyond the high school level of education. *
I understand that the program is in an Online Independent-Study Format and may include live communication by phone or through the Internet. *
I understand that a total of 3 hours Mentoring Time is included in the program. *
I understand that after the 3 hours of mentoring time is completed, I would need to purchase additional Mentoring or Coaching Services. *
I understand that I will need to have satisfactory grades on my assignments and tests for each lesson before I complete the next lesson. *
I understand that I will need to complete requirements for both Part 1 and Part 2 of the program, AND pass the Final Oral Skills Evaluation in order to receive a Certificate of Completion. *
I understand that I may need additional time beyond actual online time to complete lessons, assignments, and tests. *
I understand that I may be asked to take a Third-Party Language Assessment at my expense based on my Mentor's Assessment of my language skills. *
I understand that there may be additional requirements for specific programs or courses that are not included on this list. *
Expedited and Fast Track Options: If I sign up for Expedited Plan or Fast Track, I will complete one lesson per day based on satisfactory performance and mentor's approval.. *
Inactive Status: If I do not submit a test or assignment within a 3 week period, I will be transferred to Inactive Status and I will need my mentor's approval to continue with the course. I understand that I may need to complete and/or pass additional requirements in order to continue with the course. *
Course Extensions: I understand that access after 3 months (for the 3-month program) is not guaranteed, and requires application and approval for course extension and based on mentor's availability, program's availability, language pack availability and other factors.
I understand that I need my mentor's recommendation for Part 2 access, depending on my satisfactory completion of assignments and tests and other requirements.
I understand that the fees, requirements, and content may be revised at any time to improve quality, or to comply with Standards, or for educational, or administrative reasons. *
I understand that I may be required to participate in telephone coaching/mentoring or drills and practice sessions, or other activities, if recommended by my mentor based on my learning needs and course performance. Some or all of these additional activities may require extra fees.
If my course performance is deemed usatisfactory, I have the option of accepting or not accepting my mentor's assessment and recommendations. If I choose not to accept my mentor's assessment and recommendations, I would be required to pay a re-evaluation fee for a second opinion from another mentor and/or transfer to another mentor, if one is available for the same language pair. If I decline additional mentoring, I may request a challenge exam to demonstrate my skills and this would require additional fee. *
I understand that I am responsible for maintenance of my own computer and Internet connection and services that may be required for the program. Technical support provided by Culture Advantage is limited to support with the content, navigation, and course access.
I understand that content not owned by Culture Advantage is owned by their respective copyright owners and I agree not to not to share, post, email, copy, reproduce in any manner, or disclose to third parties without written permission from the respective copyright owners. *
I understand that all content, including the format, layout, design and presentation, except for links identified as external or third-party websites, is owned by Culture Advantage. I agree not to share, post, email, copy, rent, sell, reproduce in any manner for commercial purposes, or disclose to third parties without written permission of Culture Advantage. *
I understand that I am responsible for keeping my account current, and for paying fees when they are due. *
I understand that there may be additional fees for test retakes, review programs, personal coaching, re-assessments, for oral testing, remedial testing, for cancellations, or for additional services that are not included in my Program Plan. *
I understand that if I cancel my registration AFTER 24 hours, my fees will be refunded minus a $70.00 cancellation fee. Additionally, after the first 48 hours of access there is a 10% "access fee" charged per day upon cancellation. *
I understand that I will not receive a refund if I cancel or withdraw from the program after one week of my Start Date. *
